I’m sure it’s related to some past trauma and breaks my heart to do it to him but sometimes car rides (and vet visits) are a necessity, even if cats can’t under that. I’m sorry though, it’s rough to feel like you’ve hurt them and you can’t explain why.

Cats feel safest when their environment is consistent and predictable, and going to the vet is none of those things. There are ways to mitigate the stress of a vet, including trying to find a vet that does house visits or is trained in “stress-free handling”.
